Please fill out the form below to become a Pet Companion. A $35 application fee will be required in order to perform a background check and provide you with a t-shirt and seat belts for the pets . However, keep in mind that if you answer "no" to any of the questions, that does NOT prevent you from becoming a Pet Companion.

Do you have a valid drivers license and a car?
Are you able to walk dogs for up to an hour?
Are you open to pet boarding or day care?
Are you willing to do long distance travel, such as pet transport?
